Аннотация: Цель. Сравнительный анализ клинических результатов имплантации лекарственных и непокрытых стентов, динамики коронарного атеросклероза за 5 лет наблюдения. Материалы и методы. В исследование были включены 585 пациентов с многососудистым поражением коронарного русла (МСП КР). 321 больному с этой патологией была выполнена имплантация стентов с лекарственным покрытием (рапамицин). Контрольную группу составили 264 пациента с МСП КР которым была проведена имплантация непокрытых стентов. Эндоваскулярное лечение в одной группе стентирования выполняли с использованием лекарственных эндопротезов Cypher (активное вещество - рапамицин), а в другой - непокрытых стентов BX Velocity Были проанализированы годичные и пятилетние клинические результаты. В исследуемых группах изучали отдаленную выживаемость без основных коронарных событий (рецидив стенокардии, повторная реваскуляризация, инфаркт миокарда - ИМ, коронарная летальность), частоту развития рестеноза (РС), прогрессии атеросклероза в стентированных и других сегментах КР Результаты. Через год у пациентов с непокрытыми и лекарственными стентами рецидив стенокардии составил соответственно 22,3% и 11,8% (р < 0,05), необходимость в проведении повторного эндоваскулярного лечения -15,6% и 3,9% (р < 0,05), операции аорто-коронарного шунтирования (АКШ) - 2,8% и 0,3% (р < 0,05), развитие ИМ наблюдалось в 1,4% и 0,9%. Частота развития РС - 19,7% и 2,3% (р < 0,05), а позднего тромбоза - 0,3% и 1,4%. Выживаемость без коронарных событий была достоверно выше в группе больных с лекарственными эндопротезами. Через 5 лет у пациентов с непокрытыми и лекарственными стентами рецидив стенокардии составил соответственно 30,7% и 22,7%, необходимость в проведении повторного эндоваскулярного лечения - 23,9% и 18,1%, операции АКШ - 6,4% и 4,7%, развитие ИМ наблюдалось в 6,5% и 7,8% случаев. Частота прогрессирования атеросклероза в стентированных сегментах - 6,6% и 10,1%, а позднего тромбоза - 0,4% и 2,1%. Выживаемость без коронарных событий достоверно не различалась между группами. Выводы. К окончанию 1-го года наблюдения выживаемость без основных коронарных событий (летальность, развитие ИМ, рецидив стенокардии, повторная реваскуляризация) была достоверно выше у пациентов с лекарственными стентами. К окончанию пятилетнего периода наблюдения не отмечено достоверных различий в показателе выживаемости вследствие большего количества поздних тромбозов и прогрессии атеросклероза в группе больных с лекарственными эндопротезами.
